When writing in first person POV do I ever italicize the protagonist's thoughts or not because it's in his POV?

Whenever I write in first person, whatever isn't in quotations is technically their thoughts because they are recognizing physical actions done by others. But when they are actually thinking...gosh this is hard to explain. OK. Short answer: yes, I would.
